Description
This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number RFQ 546-94-02 award will be made using the simplified acquisition procedures part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-06. This acquisition is unrestricted. The North American Industry Classification System # 334112 and the small business are 100,000. THE SCHEDULE The Contract resulting from this solicitation shall be to provide Maintenance and Service in support of the Digital Dictation System ? Stock# RTAS 9000 located in the Radiology Department located at the V. A. Medical Center, 1201 N.W. 16th Street, Miami, FL 33125. For the period beginning April 30, 2002 Through March 1, 2003 with the option for a two (2) year period. All quotes are requested to broken down into monthly charges. Base Yr 2002 ($____________) Option Year 1 FY 2003 ($____________) Option Year 2 FY 2004 ($____________) Grand Total ($ _____________) SCOPE / STATEMENT OF WORK To provide Service and Maintenance in support of a Digital Dictation System located in the Radiology Department at the VA Medical Center, 1201 N.W. 16th Street, Miami, Fl 33125. DIGITAL DICTATION SYSTEM STOCK # RTAS 9000 Company agrees to provide 24 our telephone consultation, 365 days a year. Company agrees to provide Local on-site service to Radiology, Monday - Friday, 8:00 a.m. - 5:00 p.m., local time, except for holidays: New Year's Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve and Christmas Day. Company Service Technicians must be factory trained and certified. The Technicians that are dispatched are to arrive at Radiology within 24 hours from notification by Radiology Representative of known or suspected failure of one or more pieces of equipment covered under this agreement. Replacement parts will be furnished and installed by Company Service Personnel at no extra charge. Component parts, assemblies, or subassemblies may be replaced with new or refurbished items at the Company?s option. If parts must be replaced due to causes other than normal wear and tear, Company will charge the price in effect at the time of such parts and all reasonable expenses associated with Company's cost to replace said parts. The contractor shall provide proof of unreasonable or misuse of damaged parts. In the event that a required spare part is not locally available and the failed part cannot be repaired by the local Service Technician, parts will be shipped immediately by their supplier via 24-hour express service to arrive the next morning following notification (Monday through Saturday), contingent on such notice being provided prior to 4:00 p.m. Eastern time. When notification prior to 2:00 p.m. Saturday enables Monday a.m. Delivery. When notification is received after this time, the parts will be shipped no later than the following morning. If the Company determines it?s Service Manager that a particular replacement part is critical for the system to be operational. The Manager may decide to ship such part via the same day airline service. In the case of a weekend Emergency, notification prior to 2:00 p.m. Saturday enables Monday a.m. delivery. Company will deliver and install, at no additional cost, hardware and software Field Engineering Change Orders on the equipment covered by this under this Service Agreement. The company resolves to work toward giving our RTAS System availability approaching 100%. In order to do this, the Company, may based, on technical judgements made by the Company?s Service Personnel, request to be able to monitor machine functions via its Remote Diagnostic Facility (RDF), but always with prior knowledge, approval, and cooperation of Radiology. At such time the Company may also make changes to the resident software, but never in a way that would knowingly disrupt normal operations, violate security, or disturb Radiology?s records. The Company may, from time to time recommend and initiate replacement of suspect component at no expense to the Veterans Administration, but with planned cooperation regarding replacement work. This will always be done with every Intention of minimizing disruption. Finally, if Company deems it advisable. For a Factory Service Technician to visit and perform machine or operational remedial on-site, such a trip will be initiated by Company, but with the full knowledge and cooperation of Radiology. The full expense for such travel including per diem, living expenses and all incidental costs relating to either the trip or the service The Company resulting in no cost to Veterans Administration will pay Work. An exception to this if it is determined that the problem developed from a part damaged by causes other than normal wear and tear. The Veterans Administration will maintain the environmental conditions as specified by the requirements of the equipment.
