- Q-- ON-SITE EMPLOYEE HEALTH SERVICES SOL 222-02-2005 DUE 033102 POC Patsy Scott, Contract Specialist, 870/543-7577/Priscella Sullivan, Contracting Officer, 870/543-7271 Utilizing an Arkansas licensed physician and an Arkansas registered nurse,the contractor shall furnish qualified personnel, services and disposable supplies as necessary to conduct an on-site employee health program at the NCTR, Jefferson, Arkansas, for approximately 600 employees under a cost-plus-fixed-fee contract for a 5 year period beginning 04/30/02 through 03/31/07. Services will include, but not be limited to: physical examinations (pre-employment, pre-placement, periodic, termination and special), health appraisals, immunizations and care for illness and injury at work. This solicitation is issued for Full and Open competition. The solicitation will only be available on or about 02/25/02 on the FDA web site at http://www.fda.gov/oc/ofacs/contracts. Prospective offerors are responsible for downloading the solicitation and amendments.
