- Description
- The Carl T. Hayden VA Medical Center anticipates contracting for Community Based Residential Care, Treatment and Rehabilitative Services for chronically mentally ill homeless veterans of the Carl T. Hayden VA Medical Center, Phoenix, Arizona. Offers will be accepted only from offerors who are financially responsible. All responsible offerors may submit an offer and will be considered. It is the intention of the VA to enter into a Basic Ordering Agreement for these services. The term will include a base year anticipated for May 1, 2002 to April 30, 2003 with two (2) one (1) year renewal option years. The contract facility must pass an inspection conducted by a VA inspection team prior to award. This is a total 100% small business set-aside. The NAICSC code is 623220, size standard $5.0 applies. Contractor's proposed facility must be located in the Phoenix Metropolitan area. Solicitation documents will be available on or about March 11, 2002 at http://www.va.gov/oa&mm/busopps.htm.
