- Description
- The Federal Correctional Complex (FCC), Coleman, Florida, intends to award a non-personal service/requirements firm-fixed price type contract for Ophthalmology Services for the inmate population at the Federal Correctional Complex (FCC), Coleman, Florida. These services will be provided at the FCC, 846 NE 54th Terrace, Coleman, Florida, 33521. The Period of Performance is from date of award (approximately April 8, 2002) through September 30, 2002. The estimated quantity of Ophthalmology services will be a minimum of 1 session and a maximum of 12 sessions. A session is defined as a six (6) hour time period. Ophthalmology routine evaluation services provided within the session will be based on FCC Coleman requirements. Sessions will be scheduled on an as needed basis, usually twice a month, but sessions will not be scheduled unless there is a need to fill the entire six (6) hour session time frame. All supplies needed will be provided by the FCC, Coleman. The contractor will be responsible for evaluation of patients; counseling patients prior to any required surgical procedure; and, signed SOAP note format written reports of all patients evaluated. All records maintained by the contractor under this contract will be the property of the Government. The contractor will provide an itemized billing statement to the Contracting Officers Technical Representative (COTR) that accurately reflects the date of service, the number of evaluations provided and the total dollar amount for the service provided. The COTR for this requirement is Michael Grizzle, Health Services Administrator, FCC, Coleman, Medium facility. Special Qualifications required include, but are not limited to the following items: A graduate of an accredited University with a degree in medicine with further studies in the field of Ophthalmology and be at a minimum, eligible for Board Certification; must have a license to practice medicine in the State of Florida; must be privileged at a local hospital at the time the award is made; and, current malpractice insurance. Offerors must identify within the offer documentation, at a minimum, two (2) prospective qualified persons. Copies of all documents verifying credentials and proof of malpractice insurance must be submitted with the offer. Failure of submitted personnel to meet the requirements will result in the award to other than the lowest bidder. The anticipated date of the solicitation is March 5, 2002 (via www.fedbizopps.gov) with offers due by March 26, 2002, 3:00 P.M. local time(EST).
