- Description
- Contractor will furnish all labor, supervision, supplies and transportation necessary to perform the requirements of this contract. Contractor shall provide a certified orthotist/pedorthotist to provide diabetic shoes and insert for the Prevention-Amputation, Care and Treatment (PACT) clinic at VA Medical Center, 700 S. 19th St, Birmingham, AL 35233. The clinics are held on every Thursday, Friday and Wednesday from 7:30 am until 12:00 noon. Contract will be awarded for a base period with two (2) one (1) year options to renew. Services to begin on or about April 15, 2002. NAICS 339113. Solicitation packages will be available on or about February 12, 2002. This is a Total 100% Small Business Set-aside.
