- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-03. This procurement is 100% set aside for small business concerns. The North American Industry Classification Code is 446110. The small business size standard is $5 million. CLIN 0005A: Option Year 4, Outpatient Pharmacist Services; Est?d Hrs: 2080; Unit Price:_____; CLIN 0005B: Option Year 4, Outpatient Pharmacist Services; Est?d Hrs: 2080; Unit Price:________; Total Amount:_____. //GENERAL DESCRIPTION: The primary functions of the pharmacist will be interpretation of physician?s orders, dispensing appropriate medications and counseling patients on proper use of their medication. The pharmacist will possess the following attributes: (1) Knowledge of drugs and drug products, appropriate dosages, routes of administration, physical and chemical properties and incompatibilities; (2) Ability to communicate effectively orally and in writing with people from a variety of backgrounds; (3) Working knowledge of various programs of drug distribution, unit dose (centralized and decentralized), outpatient pharmacy, ward stock and IV additives; (4) Ability to supervise peers and technicians when needed. DRUG DISTRIBUTION AND CONROL: (1) All medication order entry and dispensing are accurate and complete. (2) Workload is prioritized so that prescription and medication orders (STAT, now, and first doses) reach patient in a timely manner. (3) Assure that preparing, dispensing of medications adheres to law, regulation, policies, procedures, professional standards of practices. (4) Evaluates physician orders for appropriateness based on review of patient medication profile. (5) Accurately enters and/or verifies data into computer system. (6) Checks work of pharmacy technicians, pharmacy students and volunteers for accuracy. (7) Dispenses completed prescription with instructions to patients. (8) Resolves all prescription problems or questions with physician prior to dispensing. PHARMACEUTICAL CARE SERVICES: (1) Medication orders are reviewed for therapeutic appropriateness (dose, schedule, route, form, indication, duplication, treatment guidelines, untreated indication policies). (2) Therapeutic drug interventions are identified, resolved and documented. (3) Patients are efficiently and professionally counseled about medications, including OTC products. (4) Provides consultation to physicians, nurses, other health care professionals on drug related matters. (5) Evaluates patient?s ability to understand medication instruction and provides oral and written counseling on drug therapy. PRACTICE MANAGEMENT: (1) Effectively use time to complete work-related duties, including supervision of technical related duties. (2) Maintains computer security/access code to Pharmacy. (3) Maintains formulary as dictated by VISN. Demonstrates initiative in recommending strategies to maximize medical center resources. (4) Performs clinic and pharmacy drug inspection. (5) Maintains records for controlled substances, narcotics. (6) Supervises subordinates and acts as pharmacist in charge when necessary. (7) Keeps supervisor informed of all pertinent problems or situations. PROGRAM SERVICES: (1) Provides accurate, timely drug information to health care providers. (2) Actively participates in medical center and pharmacy quality activities. (3) Achieves/maintains required competencies and training as a pharmacist. (4) Performs training of technicians, volunteers and pharmacy students when required. (5) Participates in patient discharge counseling and teaching when appropriate. (6) Participates in various clinical assignments. OTHER: (1) Contractor must be able to read, write and understand the English language, be willing to work rotating shifts and holidays. Holidays will be paid at the regular hourly rate. Contractor must be able to lift 30-40 pound boxes. Typing skills are required. Contractor must be able to operate the following: computer and associated hardware; prepack label machine; automatic pill counting machine; unit dose packing sealer; TPN mixer; copy machines and typewriter. Contractor personnel must be willing to submit to a drug screening urinalysis if selected. Contractor must have following insurance and provide certificate: Error and omission, $1,000,000; Comprehensive general liability, $2,000,000; Bonding, $25,000; Workers? compensation, Statutory rate. EVALUATION FACTORS: Technical (Qualifications of Pharmacists); Management (Availability of Candidates, Past Performance, Approach to Providing Services); and Price. Technical and Management, when added together, are substantially more important than price. TERM OF THE CONTRACT: One base period from date of award through September 30, 2002, with the option to extend for four (4) additional one year periods. QUALIFICATIONS: Personnel assigned by contractor to perform services of this contract shall be a Registered Pharmacist in any state or District of Columbia. Must have a minimum of a BS in Pharmacy. MONITORING/RECORD KEEPING: The contractor must establish a tracking system of hours worked. The payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. The Contracting Officer?s Technical Representative (COTR) will monitor contractor employee work hours/services. The COTR contact information will be provided with the notice of award letter. Documentation of services will be reviewed prior to certifying payment. VASLCHCS will pay only for services actually performed at the VASLCHCS and in strict accordance with the pricing shown in the contract. The contractor may not certify invoices for payment. WORK HOURS: Inpatient?Monday through Friday, 12 Midnight to 8 AM; Outpatient?Monday through Friday, 8:30 AM to 5 PM, to include all holidays. HOLIDAYS: The ten holidays observed by the Federal Government are: New Year?s Day; Martin Luther King Jr.?s Birthday; President?s Day; Memorial Day; Independence Day; Labor Day; Columbus Day; Veterans Day; Thanksgiving Day; Christmas Day. PERSONNEL POLICY: The contractor shall provide the following for personnel furnishing services under this contract: Workers? Compensation; Professional liability insurance; income tax withholding; and Social Security payments. OFFERORS ARE REQUIRED TO INCLUDE A COMPLETED COPY of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items ALT III (OCT 2000), with its offer. The following provisions apply to this acquisition: 52.212-1; 52.212-2, 52.217-8, 52.217-9, 52.237-7; 52.217-5. The clause at 52.212-4, Contract Terms and Conditions - Commercial Items (AUG 2000), applies to this acquisition with the following clauses added as addenda to the clause: 52.204-4, 52.224-1, 52.224-2, 52.225-3, 52.225-13, 52.225-16, 52.232-4, 852.270-4, 852.237-70, 852.270-1, 852.237-7 (which requires submission of certification of indemnification/liability insurance, consistent with the laws of the state of Utah).
