- Contact Mr. Jay Windy Boy, Purchasing Agent, (406) 247-7064 or Mr. Jerry Black, Senior Contracting Officer at (406) 247-7068. A&E services are required for Metes and Bounds Surveying on the various Indian reservations throughout the State of Montana. They include the Blackfeet Indian Reservation, the Crow Indian Reservation and the Fort Belknap Indian Reservation. These surveys will define the property owned or leased by the United States Government, Department of Health and Human Services. The metes and bounds survey will require concrete monuments with bronze identifying caps at all angle points or changes of direction. DOCUMENTATION: Each survey site will require a Certificate of Survey that includes at a minimum: Purpose of Survey; legal description; a detailed land survey plat (at 1-inch to 100-foot scale); Notice of Easements; Notice of Right-of-Way; signature and State registration number of surveyor; and a seal from a State Notary Public Official. Other requirements may be needed per local and State laws and regulations. This acquisition is set-aside to 51% Native American owned business concerns in accordance with the Buy Indian Act. The standard industrial classification code is 7389/8713, engineering services. The contractor's annual receipts for the last three (3) preceding years shall not exceed $2.5 million. Firms responding to this announcement before the closing date will be considered for selection, subject to any limitations indicated with respect to size and geographic location of firm, specialized technical expertise or other requirements listed. Following an initial evaluation of the qualification and performance data submitted, three or more firms that are considered to be the most highly qualified to provide the type of services required, will be chosen for negotiation. Each firm shall be evaluated on the following criteria: (1.) Professional qualifications necessary for satisfactory performance of required services; (2.) Specialized experience and familiarity of current regulations and evidence of similar completed jobs; (3) Capacity to accomplish the work in the required time frame; (4.) Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work and compliance with performance schedules and (5.) Location in the general geographical area of the project and knowledge of the locality of the project. Firms that meet the requirements described in this announcement are invited to submit (1.) a letter of interest and a statement that the firm is a small business concern; (2.) a Standard Form 254, A/E and Related Services Questionnaire for the firm and all consultants; and (3.) Standard Form 255, A/E and Related Services Questionnaire for specific projects. Submit them to the USPHS Indian Health Service, Mr. Jay Windy Boy, Purchasing Agent, P.O. Box 36600, Billings, Montana 59107-6600 by the close of businesses on Tuesday, February 19, 2002. The estimated price range for this acquisition is between $25,000 and $100,000. The completion time for this firm fixed price acquisition is sixty- (60) calendar days.
