Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6., as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for quotes are being requested. THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is RFQ-797-FDF2-02-0013. The Air Force Medical Logistics Office, Veterans Affairs Special Services has a requirement for the following: Quantity of 10 each Portable Oxygen System, part number 9320116-1; quantity of 1 each Test Stand, part number 8820061-1; quantity of 2 each Adapter Assembly, part number T80-3007-9; and quantity of 10 each Hose Assembly, part number 7920031-9. These products are manufactured by Carlton Technologies. Delivery, which shall be FOB DESTINATION ONLY, shall occur 30 calendar days after award of the purchase order. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror--Commercial Items. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror--Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror?s initial offer should contain the offeror?s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. Offers are due by 4:00pm EST on 11 February 2002 at AFMLO/VASS to the address shown in this synopsis. Offers may be faxed to Monica Pileggi at 301-619-6645.
