- Description
- Provide Ophthalmology services for a period of one year from date of award with option to extend for two additional years. Services include ophthalmology procedures and approximately 48 outpatient clinics during the contract period. All responsible sources may submit a proposal which shall be considered by the agency. RFP will be available January 24, 2002 and if not downloaded from the internet must be requested in writing FAX 970-244-7721. Proposal is due February 26, 2002. NAICS for this procurement is 62111.
