- NA This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number DADA13-02-T-0011 applies and is issued as a Request for Quote (RFQ). Solicitation Line Items are as follows: Line Item 0001, RETCAM 120, Wide Field Digital Imaging , P/N 10-00-002), 'or equal', 1 ea. Line Item 0002, Special ROP Lens, P/N 01-00-015, 'or equal', 1 ea. Line Item 0003, Mitsubishi High Resolution Dye Printer, prints 6? x 4? high resolution pictures, P/N 03-06-033, 'or equal', 1 ea. The system will be used at Madigan Army Medical Center to photographically document ocular pathology (retinal, optic nerve) in children. The system shall include: standard children lens, portrait lens, image capture unit, light control unit, mobile cart, image processing unit, w/17? monitor, proprietary software (latest revision), uninterruptible power supply and on site installation and training. The solicitation document and incor porated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. This solicitation is issued as 100% Small Business Set Aside with an NAICS Code of 54171 and size standard of 500. The following clauses apply to this solicita tion: FAR 52.212-1, Instructions to Offerors?Commercial Items; however, sub-paragraphs (g) and (h) are deleted as this is an RFQ. FAR 52.212-2, Evaluations, Commercial Items with the following factors included in FAR 52.212-2(a) used to evaluate offers: T echnical Capability, Past Performance, and Price. Technical Capability and Past Performance, when combined are significantly more important than price. Offerors shall submit a statement of their capability as it relates to the requirements set forth in t he specifications; a minimum of 2 references with whom past performance can be verified to include points of contact, telephone numbers and a total contract price. FAR 52.211-6, Brand Name or Equal applies to this solicitation. Offerors shall submit as a part of their quote, completed copies of the provisions at FAR 52.212-3 with Alt I, Offeror Representations and Certifications?Commercial Items and DFARS 252.212-7000, Offeror Representations and Certifications?Commercial Items. FAR 52.212-4, Contract Te rms and Conditions?Commercial Items, applies to this acquisition; addenda are as stated herein. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items applies to this acquisition. Offers in response to this combined synopsis/solicitation are due not later than 18 Dec 01, 4:00 PM Pacific Time. Once the Contracting Officer has decided with whom a contract will be entered, a bilateral contract requiring the quoter?s signature will be issued. A contract will be in existence once both the quoter and the Contracting Officer have signed the document. Wherever the words 'offe', 'proposal', 'offerors', or similar te rms are used in this solicitation, they shall be read to mean 'quote', 'quotation, or 'quoter or similar corresponding terms to reflect that this solicitation is a Request for Quote. Firms offering in response to this notice should ensure registration in the Central Contractor Registration Database, HTTP://WWW.CCR.GOV, prior to submission for consideration. No contract award will be made until appropriated funds are made available from which payment for contract purposes can be made.
