- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. Solicitation number for this RFQ: 672-228-02 is a Request for Quotation. The following Provisions and Clauses are incorporated into this RFQ by reference (FAR clause website is www.arnet.gov/far/ .) and are in effect through Federal Acquisition Circular 97-27 June 2001. No addenda apply to these Provisions and Clauses. FAR 52.212, Instructions to Offeror -Commercial Items, 52.212-3,Offeror Representations and Certifications-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Applicable Clauses in FAR 52.212-5 paragraph (a) are 52.222-3,52.233-3 and in paragraph (b) are: 52.222-21, 52.222.26, 52.222-35, 52.222-36,52.222-37,52.222-19,52.225-3, 52.225-13, 52.225-15, 52.225-16, 52.232-33, 52.232-34, 52.247-64. The offerors will include a completed copy of provisions FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. No additional FAR clauses apply. Offerors must certify in FAR 52.212-3(i) that the Company and/or any of its principals is/is not on the list of Parties Excluded form Federal Procurement or Non-Procurement Programs (http://www.arnet.gov/epls/. This requirement is 100% set-aside for Small Business. North American Industry Classification System (NAICS) 421450, Size Standard $5.0. VAMC requirement is: CLIN 001: Fifteen (15) Low Air loss Therapeutic Support Surface with the following features: CPR deflate, panel lockout, low pressure alarm, twenty (24) hour backup; lateral rotation which automatically turns patients to 40 degrees to reduce risk of pulmonary and other serious medical complications; low air loss pressure relief for stage II pressure ulcers; user-friendly controls; lightweight mattress replacement system that fits most standard hospital beds. Specifications: Electrical Power leakage current 115vac, 50/60hz, 2A; Dimensions for Standard Mattress: 84?L x 37? W x 8?; Weight: 25lbs; Control Unit: 30lbs; Maximum Patient Weight 500lbs; Safety Code Approval. Evaluation factor-lowest price including Freight on Board (FOB) destination Door to Door and delivery date Forty-Five (45) days from date of contract award.Offerors shall state their standard warranty they offer in this response to this RFQ. Award will be based on lowest price FOB: Destination (Door to Door) for this equipment. The Government contemplates award of a Firm Fixed-Price contract resulting from this solicitation. All responsible sources may submit a quotation that will be considered by the Purchasing Agent or Contracting Officer. Quotations must be submitted via mail to VA Medical Center, 10 Calle Casia, San Juan, PR 00921-3201 (if mailed via FEDEX the zip code must change to 00927). Facsimile quotations will be accepted and may be sent to 787-641-8364. Offers shall be submitted on Company letterhead (see FAR 52.212-1(a) 1-11).
